How does the program work?
The Homelessness Health Support Team (HHST) Program supports people who are experiencing homelessness, or at risk of experiencing homelessness including sleeping rough, couch surfing and living in overcrowded dwellings to access the healthcare they need.
Participants will be referred to a HHST Program team member (care coordinator, service navigator or care worker) who will work closely with them to develop individualised health and wellbeing goals. They will receive person-centred support to access the right healthcare services and local support to enhance their overall wellbeing.
The services may include:
- Working with the participant to better understand their health needs and plan their care
- Health coaching to build participant’s understanding of their own health and wellbeing needs, including support for lifestyle changes
- Guidance to navigate the healthcare system
- Support to access community-based services to minimise avoidable hospital admissions
- Help to access Government funding and assistance schemes
- Assistance to establish connections with local supports and networks

- Greater Hume Shire
- Edward River
How do I refer to the program?
GP referrals are not necessary –anyone can make a referral – just give our friendly team a call if you or someone you know could do with some support.
This service is supported by funding from the Murrumbidgee PHN through the Australian Government’s PHN program.
