Stronger Together Two

What is the Stronger Together Two program?

This program has been funded by the NSW Government Ageing, Disability and Home Care Department (ADHC), to provide services for people with a disability from 0 to 65 years living at home, with family, carers or on their own. The disabilities are a vast range from intellectual to physical.


The main outcome of the Stronger Together Two (ST2) program is to help the clients and their families be independent and knowledgeable about mainstream services and continue to be able to support and live as comfortably and independently as possible to enable the clients to stay at home.

What does a Support and Capacity Development Coordinator do?

Referrals for the ST2 Program are made through ADHC, please contact the Information, Referral and Intake team on 1300 134 450. Family members, a person with a disability or a worker from another service can contact ADHC to discuss eligibility for a referral.

 The Support and Capacity Development Coordinator will assist with:

  • Person centred planning
  • Links and integration to the mainstream community
  • Ongoing self-management through the service system
  • Advocacy when dealing with service providers
  • Information and referrals to access available funding and resources
  • Support and linking carers to support groups
  • And the list goes on…
